接纳与承诺疗法(ACT)与佛学

摘要:接纳与承诺疗法(Acceptance and Commitment Therapy, ACT)是新一代认知行为疗法(Contextual Cognitive Behavioral Therapy, cCBT)中最为代表性的经验性行为治疗方法。ACT以功能性语境主义为哲学背景,以关系框架理论(Relational Frame Theory,RFT)为理论基础,认为人类主要的心理问题源于语言/认知与我们的生活环境之间的交互作用方式,导致人们有悖于长期价值观的行为及心理上的不灵活。ACT通过正念、接纳、认知解离、以自我为背景、明确价值和承诺行动等过程以及灵活多样的治疗技术,帮助来访者增强心理灵活性,投入有价值、有意义的生活。ACT从创立到应用都与东方禅宗的一些观念相关,本文对ACT疗法的哲学背景和理论基础、心理病理机制、治疗模型,以及与佛学的相关性进行了综述,以供国内心理治疗学界同行更好地理解这一新的认知行为疗法,并了解认知行为治疗第三浪潮的意义和价值。

Acceptance and Commitment Therapy(ACT)and Buddhism

Abstract: Acceptance and Commitment Therapy (ACT) is one of the most representative experiential behavior therapies in the new generation of Cognitive and Behavior Therapy, named as “contextual cognitive behavioral therapy”. The theoretical foundation of ACT is Relational Frame Theory (RFT) based on the philosophy of functional contextualism, which suggests that the main psychological problem derived from verbal and cognitive interaction with environment, leading to behaviors contrary to long-term values and psychological inflexibility. ACT aims to enhance the psychological flexibility in order to help clients devote to valuable and meaningful live, with mindfulness, acceptance, defusion, self as context,value and committed action as well as flexible treatment techniques. We reviewed the philosophy background, theoretical foundation, psychopathology, therapeutic model, and relationship with Buddhism, so as to promote a better understanding of the new cognitive and behavior therapy as well as the significance and value of the relationship of the third wave of cognitive and behavior therapy with Buddhism.

1 前言
接纳与承诺疗法(acceptance and commitment therapy,ACT,作为一个词读,而不是分开的A-C-T)[1],由美国内华达州大学心理学教授Steven C. Hayes博士及其同事于20世纪末21世纪初所创立,是一种以有关人类语言、认知的关系框架理论和功能性语境主义哲学为基础的经验性行为心理治疗,采用接纳、认知解离、正念、观察自我以及价值、承诺行为等改变过程以创造心理灵活性(psychological flexibility)[2]。ACT将人类心理问题的核心根源归纳为四个方面,缩写为FEAR。分别是思维融合(Fusion with thoughts),经验评价(Evaluation of experience),经验回避(Avoidance of experience)和行为解释(Reason-giving for behavior)。而个体保持心理健康的策略是三点,缩写为ACT:接纳反应和正念(Accept reactions and mindfulness),选择符合自己价值观的方向(Choose a valued direction),做出行动(Take action)。接纳与承诺疗法(ACT)是认知行为治疗“第三浪潮”中最为核心的经验性行为心理治疗[3]。本文对ACT疗法的哲学背景和理论基础、心理病理机制、治疗模式,以及与佛学的相关性进行了初步探讨,以抛砖引玉,供国内心理治疗学界同行更好地理解这一新兴的认知行为疗法,并了解新一代认知行为治疗与佛学相关的意义和价值。
2 接纳与承诺疗法的理论背景
2.1 ACT的哲学背景:功能性语境主义(Functional Contextualism)
ACT起源于行为分析(behavior analysis),现代行为分析以“功能性语境主义”作为基础[4]。语境主义把心理事件理解成整体与具体情境(situation)(包括历史和环境)持续不断的相互作用,寻求保持与整个心理事件及其情境之间的联系,并以其整体性不遭破坏的方式分析心理事件,并试图预测和影响整体与具体情境之间的连续互动,力求使分析达到一定的精确度、范围和深度[4, 5]。相对于心理事件的形式,语境主义对心理事件的功能最为关注。语境主义认为,从某种程度上说,任何脱离历史和当前情境的事件都不能称之为“事件”;同样,行为也应该与具体的情境密切相关。
