DSM-5中人格障礙(Personality Disorders)之主要改變
The Major Changes of Personality Disorders in DSM-5

人格障礙診斷標準的改變,堪稱是DSM-5試圖創新地結合類別的(categorical)與層面的(dimensional) 概念而成的混合(hybrid)模式中的指標性診斷之一。美國精神醫學會於2011年7月7日的新聞稿【1】中指出,「這次的改變將障礙類別(disorder types)及病態人格特質(pathological personality traits),更重要的是,與人格運作( personality functioning)的減損程度(levels of impairment)整合在一起。」如同其它診斷標準一般,這項草案目前也正在美國各地進行田野試驗(field trials)中,參與第一回合田野調查的有將近一打的大型學術或臨床中心,近三千九百位心理衛生工作專業人士。
如此重大的改變,不免也引發美國精神醫學界許多重量級學者的善意批評,為讓讀者能以個人角度來評估將行的改變的優劣,本文將先概略介紹新的診斷標準草案,再引介前述學者們的論點以供參考比較。
由Skodol AE為首等六位精神科醫師,加上五位臨床心理學家所組成的人格障礙工作小組從2007年開始運作。在美國精神醫學會網站上展示的2011年6月21日的最新診斷標準【2】可見,其中有一個改版的一般準則(general criteria),診斷人格障礙的必要條件是同時存在人格運作 (自體《self》及人際的《interpersonal》)的減損及病態的人格特質。也就是說,要符合下列診斷準則:
A. 在自體(認同感《identity》或自我指向《self-direction》)及人際的(同理心《empathy》或親密感《intimacy》)的運作上有顯著的減損。
B. 一或多種領域 (domain) 或面向(facet) 的病態性人格特質。
C. 在人格運作或人格特質呈現(expression)上的減損,無論在時間上和不同情境 中,都是相對穩定的。
D. 在人格運作或人格特質呈現上的減損,不能僅由個人發展階段或社會-文化環境的常態(norm)來理解。
E. 在人格運作或人格特質呈現上的減損,並不光來自某種物質(如,濫用的藥物)或一種一般性醫療狀況(如,嚴重頭部創傷)。
五個診斷準則中C.及E.延續前兩版的診斷準則,D.則是DSM-5對常模概念的表達,A.及B.則是創新,以下再略加說明:A. 診斷準則強調的是,罹患人格障礙必須自體及人際上都出現顯著減損【3】。
自尊是穩定且精確的,有能力去經歷與調節廣泛的情感經驗。後者則是能夠追尋有意義的短期的,和人生的目標,能遵守建設性的、符合社會規範的(prosocial)內在行為標準,及有成效地自我反省。
1. 自體:包括兩個成分認同整合(identity integration)及自我指向。前者指的是一個人經驗到自己是獨特的,在人我之間有著清楚的界線,隨著時間經過,對個人過去史的感受是一致的(coherent),自我評價(self-appraisal)及2. 人際:包括同理心及親密感兩個成分。前者指的是有能力去理解及欣賞他人的經驗,容忍不同的觀點,和瞭解社會因果關係(social causality)。後者則是能夠深入且持續地聯結他人,意想且有能力親近他人,在人際行為中能表達相互的關懷。
B. 診斷準則是另一個創新的層面(dimention)概念,透過心理計量學(psychometry)的評估,試圖改善前兩版的一些不足。要符合這個診斷準則至少要出現一或更多領域的病態人格特質:
1. 負向情感(negative affectivity)
2. 疏離(detachment)
3. 對立(antagonism)
4. 失控或強制(disinhibition vs. compulsivity)
5. 精神病質(psychoticism)
而且,每一領域又有三到九個不等的面向待評估。對於一般臨床工作人員來說,其實不是很容易理解或掌握的。感興趣的讀者不妨參閱 Brief Rationale and Status of the Development of a Trait Dimensional Diagnostic System for Personality Disorder in DSM-5一文【4】。
少為六個,包括,antisocial, avoidant, borderline, narcissistic, obsessive/compulsive 及 schizotypal。每個診斷類型則分別套用一般診斷準則,訂出各自的診斷標準。限於篇幅無法一一介紹,僅列舉Table 1的borderline人格障礙診斷準則,以供參考。DSM-IV的其它類型和未特定型(not otherwise specified),則將以各自最明顯的描述性特徵為基礎,而由人格運作的顯著減損狀況,結合其特定病態人格特質的方式來呈現。
更複雜的是,DSM-5又提出三種方式來量化不同診斷準則的符合程度:
一、人格運作:
亦即對自體及人際的兩者各自減損程度的評估,由健康運作(healthy functioning, Level = 0) 到極度減損( extreme impairment, Level = 4)分為五級。
二、人格障礙類型及人格特質領域:
1. 人格障礙類型:由非常符合(very good match, Level = 5)到不符合(no match, Level = 1) 分為五級。
2. 人格特質領域 : 由極盡描述(Extremely descriptive, Level = 3)到甚少(very little or not at all descriptive, Level = 0) 分為四級。
對於習慣用原型適配(prototype matching)方式來診斷疾病的許多臨床工作人員來說,新的診斷標準恐怕一時難以理解與接受。美國精神醫學會會刊在二零一零年九月號就刊出由Shedler J執筆的一篇對新診斷標準的評論【5】,其中主要的論點包括:
一、倡議中的診斷系統太過複雜,採用人格特質為基礎的診斷方式並無足夠的臨床合理性,且未經實證研究檢驗,卻又刪除一些具臨床重要性的診斷類型。
二、有足夠理由懷疑以人格特質為基礎的診斷系統即便經驗證,是否能夠轉換為臨床實用的診斷系統。
三、在創新DSM-5診斷系統時,應當體認到臨床工作人員普遍認為,比起現行DSM-IV系統,層面的特質取向(dimensional trait approaches)顯得更不相關或無用,甚至更糟。
四、倡議中的層面性特質評量方式並無法輕易呈現那些將被刪除的診斷類型,而那些診斷類型卻已在過去數十年來累積相當多的實證資料,具有顯著臨床意義的。
五、另有一些人格障礙診斷之所以被刪除,是由於沒有足夠的研究支持,但缺乏證據並不應是不存在的理由。
六、將被普遍認同的臨床架構,如嚴重度,加以層面化是值得擁護的,但採用來自學院人格心理學(academic personality psychology)的非臨床概念則是無法被接受的。
這篇評論發表後,Skodol AE旋即於2011年1月號中回應,他認為:
一、由臨床工作人員的時間、資訊和專業度來考慮,新的模式具有彈性,聚焦於人格精神病理學,且特異性(specificity)也跟著增加。
二、對(人格運作減損)程度的評量,幫助臨床工作人員判斷病人是否罹患某種與人格相關的問題,若是的話,有多嚴重?對類型的評量,則可以由廣義的描述中來表徵人格問題。對特質的評量,能更進一步地由病人特定的特質輪廓(patient-specific trait profile) 描述任一(人格)類型的異質性(heterogeneity)。想要的話,也可以描述那些與提議中的任一類型均不符合的病人,稱做特質特定的人格障礙 (personality disorder trait-specified) ,也就是先前的未特定型。
Shedler J等人立即再回應:
一、層面性特質模式是有用的研究工具,但與臨床實務的相關與效用卻是有限度;不應被用來取代臨床慣用的人格原型(prototypes),也不應被用來與以原型為基礎的診斷系統相競爭或將之複雜化。
二、學院派人格研究關注的是,全人口中不同變項之間的關係為何?層面性特質模式有助於回得這類質問。臨床實務工作需要理解一個病人心理歷程之間(psychological processes)的相關性,人格原型模式則可促進這種理解。任一種模式並不比另一種更顯科學,答案其實決定於提問。
在此之後,餘波繼續蕩漾,而且陸續已有田野試驗的論文出刊,值得臺灣學界同仁們進一步的觀察與學習。

A. Significant impairments in personality functioning manifest by:
b. Self-direction: Instability in goals, aspirations, values, or career plans.
b. Intimacy: Intense, unstable, and conflicted close relationships, marked by mistrust, neediness, and anxious preoccupation with real or imagined abandonment; close relationships often viewed in extremes of idealization and devaluation and alternating between over involvement and withdrawal.

d. Depressivity: Frequent feelings of being down, miserable, and/or hopeless; difficulty recovering from such moods; pessimism about the future; pervasive shame; feeling of inferior self-worth; thoughts of suicide and suicidal behavior.
b. Risk taking: Engagement in dangerous, risky, and potentially self-damaging activities, unnecessarily and without regard to consequences; lack of concern for one's limitations and denial of the reality of personal danger.
D. The impairments in personality functioning and the individual's personality trait expression are not better understood as normative for the individual's developmental stage or sociocultural environment.
E. The impairments in personality functioning and the individual's personality trait expression are not solely due to the direct physiological effects of a substance (e.g., a drug of abuse, medication) or a general medical condition (e.g., severe head trauma).
